Abstract
Introduction. The DM of 16 February 2018 established the modalities to pay back the cost of innovative medicines, including that the purchase is carried out by the regional public health service, even if the therapy is provided by accredited private institutions. The work aims to give an overview of the methods used by the regions/ ASLs to manage the process, as well as to quantify in economic terms the resources used by the NHS to support it. Materials and methods. The work consisted of five stages: collection of regional documents; definition of the scope of action; analysis of documentation and drafting of the questionnaire; sending questionnaires; processing the results. To quantify the costs, the working hours of the different professionals were considered and multiplied by the hourly cost (MEF annual accounts). Results. Five regions take part to the study: Veneto, Lombardia, Piemonte, Puglia, Sicilia. Regione Lazio only sent the documents. The procurement time for new innovative medicines is about 10 days, while orders from the private hospital are sent from a monthly minimum to an annual maximum. Orders from the private to the public are made by e-mail (four public institutions) or through the ERP (three public institutions), with weekly to monthly timelines. Transports responsibility is of the supplier company, which delivers to the private hospital, with the exception of Veneto and Lazio, where delivery is always to the public. Inventories are carried out on a monthly to annual basis and are managed by the public in Lazio and in Sicilia or by private in Lombardia, Veneto (Piemonte not specified). ASL Roma 2, ASP Palermo, ASP Catania ASL TO5 carry out inspections at the accredited private hospitals. The management of residues is not very procedural. The reimbursement of the medicines is the responsibility of the private except in Veneto, ASP Palermo and ASL Bari; the compilation of the AIFA monitoring registers is the responsibility of the private ent
